Late Arrival Policy:

Patients are asked to arrive to their appointments 15 minutes before their scheduled appointment time. This allows enough time for the registration process to be completed before the actual appointment time.

grace period of 10 minutes will be permitted for unforeseen delays a patient may encounter while travelling to the clinic location for their appointment. If a patient arrives more than 10 minutes late for their appointment, the patient will have to be rescheduled for a later date. This process will ensure patients that do arrive on time are seen in a timely manner. A $15 No Show/Late fee will be applied for all missed appointments. We appreciate your patience with our late arrival policy.


No Show Policy:

Your care team at Legacy Health Management Group strives to provide comprehensive, compassionate, and timely care to every patient. To enable a timely clinic flow for all patients, we ask that you notify us at least 24 hours in advance if you need to reschedule or cancel your clinic appointment.

We understand that there are times where you may encounter an emergency and miss a scheduled appointment. We do ask that – as much as possible – LHMG is notified of any potential conflicts or changes in your schedule. It is important to us that we balance our clinical teams to support all the patients within the office.


At LHMG, we do have a patient appointment No-Show/Cancellation policy. Key guidelines of this policy include:

  • Patients are expected to provide 24-hour notice of any appointment that needs to be rescheduled or canceled.
  • Patients who arrive more than 10 minutes after his or her scheduled appointment may be rescheduled to another day.
  • Patients can be dismissed from LHMG for 3 consecutive no shows.


Payment Policy:

All charges for services rendered are due and payable in full at the time of service. We accept cash, card, or money order.
